1.US passport (Current and if PIO issued on an older passport, that passport copy as well is required)

2.  If ever held Indian passport, Indian passport and Naturalization certificate 

3.  Proof of address (Lease copy, driver's license, or utility bill)

4.  2 passport size photographs     (2x2, white background). We do take pictures in our office if you would like our service.

5.  Parents' Passports and Marriage Certificate (if holding Indian passport, then proof of valid status)

6. Birth Certificate if born outside of India

7. PIO CARD
