1. US Passport (Current and if PIO issued on an older passport, that passport copy as well is required)

2. Original PIO Card

3. Indian Passport and Renunciation Certificate

4.5. If married: 

    a) Spouse’s passport 

    b) Spouse’s valid status

    c) Marriage Certificate 

5. Naturalization Certificate

6. Proof of address (Lease copy, driver's license, or utility bill)

7. 2 Passport Size Photos (2x2, white background). We do take pictures in our office if you would like our service.

VisaCare Service Charges: $150 per application (If remote service, $250)
