3.    Proof of address (Lease copy, driver's license, or utility bill)

4.    Four Passport Size Photographs (2x2, white background). We do take pictures in our office if you would like our service.

5. If married: 

    a) Spouse’s passport 

    b) Spouse’s valid status

    c) Marriage Certificate
